Treatment Adherence Intervention
YAP's Treatment Adherence Intervention ensures that clients have access to their medication, provides education and counseling around medication adherence, and helps navigate pharmaceutical logistics and medical contexts. Our goal is to help clients achieve an undetectable viral load and allow clients to live a happier, healthier, more independent life.
The Treatment Adherence Intervention includes a range of services, including:
We are committed to developing individualized, responsive, and appropriate treatment plans for each of our clients, so we will meet with potential clients to develop a plan that works for each person. Generally, the Treatment Adherence Intervention is nine months long, but we will work with clients as long as necessary to achieve viral suppression.
The Treatment Adherence Intervention includes a range of services, including:
- Assistance coordinating doctors and lab appointments
- Text reminders for medication
- Weekly check-ins
- Education and counseling
- Insurance navigation
- Assistance finding and applying for additional aid
