Treatment Adherence

Clients are typically enrolled in the Treatment Adherence program for nine months, but clients are welcome to stay as long as needed to achieve viral suppression. The program contains the following components:
  1. Intake: The streamlined intake process can take anywhere between 30 minutes and two hours, depending on the client's goals, questions, and ideas. We will coordinate an intake meeting at a time and place that is easiest for clients.
  2. Weekly Medication Reminders: You will receive individualized care and check ins from YAP staff, who will ensure that you have access to you medications and can take your medication daily. 
  3. 3-Month Check-in: You will get to have a one-on-one meeting with YAP staff to talk about your progress and make any changes needed to support your treatment adherence.
  4. Graduation: After you have achieved viral suppression and when you have easy access to your prescription, you can graduate from the program! Graduation will include compensation for your time in the program. 
We recognize that everyone approaches Treatment Adherence differently, based on your goals, challenges, and unique circumstances. As a result, we will work with you to find a plan that best fits your lifestyle. Additionally, our doors are always open--if you graduate but would like to have further support for your treatment adherence, you are welcome to rejoin the program.
